Privacy Policy - your privacy matters

Our privacy policy "Notice of Privacy Practices" provides information that federal law requires us to give our patients regarding our privacy practices. We would like you to review this document carefully and let us know if you have any questions or concerns. If you would like a copy for your records we have them available in our office for your convenience. This form also allows us to release your dental records to your general dentist and needs to be signed prior to treatment.
